Age: Must be at least 11 - 21.

KritMorph: What Kritmon you can morph into when you find out you can morph.
Kritmon Form: What do they look like in Kritmon form?

History: How you pretty much lived your life, and how you came to be captured by the mad scientist. 5-7 sentences or more.
Power: What power does your Kritmon have?
Other: Anything else you want to add about your character?
